Stretching of glomerular afferent arterioles in the swollen renal allograft undergoing acute rejection.

چکیده
Marked renal swelling occurs in acute renal allograft rejection. The impaired renal function in this state could be related in part to mechanical adjustments that the renal tubular and vascular network must make as they participate in the overall expansion of renal parenchyma! volume. In the present study we examined whether vascular stretching occurs in the swollen acutely rejecting kidney. From each of five pairs of dogs matched for weight, a donor animal was chosen from which a control kidney was removed, weighed, and perfused at constant pressure (150 mm Hg) with polymerizing silicone rubber. The contralateral kidney was transplanted into the recipient dog which was then bilaterally nephrectomized. On the sixth posttransplant day, the transplant kidney was removed, weighed, and perfused with silicone rubber, as above. Multiple coronal sections were made of each control and transplant kidney and a total of 240 photomicrographs were taken of the coronal sections from unselected fields of outer and inner cortex. The photographic slides were then coded, randomized, and projected. Measurements were then made, from the projected images, of the glomerular afferent arteriolar length (AL), and width (AW), and the glomerular width (GW). The code was then broken and the measurements in the control and transplant kidneys collated. We found that transplant AL was significantly longer (mean ratio: transplant/control •= 1.32 ± 0.06, P < 0.001) and transplant AW significantly narrower (mean ratio: transplant/control 0.88 ± 0.04, P< 0.05). We also found that GW was slightly but significantly decreased in the transplant compared to the control kidney. We conclude that afferent arteriolar stretching occurs in swollen renal allografts and accounts, at least in part, for the impaired renal function observed in acutely rejecting renal allografts. Comparison of Serum Cystatin C and Creati-nine Levels to Evaluate Early Renal Function after Kidney Transplantation
Background: Accurate and rapid assessment of allograft function is essential in renal transplant recipients in order to detect allograft rejection and to monitor drug nephrotoxicity. We aimed to evaluate the usefulness of cystatin C as a marker of kidney allograft function in the early post-transplant period and to compare this value with that of conventional serum creatinine concentration. Met...

Background: In addition to Human Leukocyte Antigens (HLA) compatibility, gene polymorphisms in cytokines might also be important in the quality of allogeneic immune response. Objective: To evaluate the influence of HLA-DR matching and a number of cytokine gene polymorphisms on acute rejection after living-unrelated donor (LURD) kidney transplantation. Methods: A total of 42 renal transplants pe...
متن کاملBENEFICIAL EFFECT OF LOW DOSE CYCLOSPORINE WITH MMF (MYCOPHENOLATE MOFETIL) IN RENAL ALLOGRAFT RECIPIENTS
Background: Calcineurin inhibitors (CNI) have significantly reduced the incidence of acute rejection. Nephrotoxicity however may contribute to long-term allograft dysfunction. Mycophenolate mofetil (MMF) may allow cyclosporine (CsA) dose reduction without increasing the risk of rejection. Methods: In seventy-eight living unrelated kidney transplant patients with renal dysfunction, we studi...
متن کاملGlomerular thrombi in renal allografts associated with cyclosporin treatment.
We have found glomerular capillary thrombi or afferent arteriolar thrombosis in eight renal biopsy specimens from seven renal allograft recipients. All patients were receiving cyclosporin and prednisolone. Biopsies were performed either routinely one and four weeks after transplantation or during periods of renal dysfunction. None of the patients whose biopsy material contained glomerular throm...
متن کاملMicrochimerism and Renal Transplantation: Doubt Still Persists
Background: The Presence of donor leukocytes in recipients of organ allograft has been shown even several years after transplantation. However, it remains unclear whether this donor cell microchimerism plays an effective role in allograft acceptance or is simply a consequence of immunosuppressive conditions in recipients. Objective: To study microchimerism in a group of kidney transplant recipi...
